Important Enrollment Requirement
Extended protection coverage, software update eligibility, and related CAD/CAM Care™ benefits are available only for systems enrolled at the time of original purchase and installation.
To maintain coverage and eligibility, membership must remain active and continuous without interruption.
Systems not enrolled at the time of purchase and installation may be ineligible for CAD/CAM Care™ benefits. Any exception, if approved by CAD/CAM Center of Miami, may require a system inspection and payment of retroactive membership fees calculated from the original installation date.
CAD/CAM Center of Miami reserves the right to determine eligibility for enrollment and continued participation in the CAD/CAM Care™ program.
